Louis Pasteur inició la teoría microbiana de la enfermedad en el siglo XIX. Lord Lister, por su parte, promovió la idea de la antisepsia, mientras que Semmelweis se centró en el lavado de manos, identificando la propagación de la infección mediante objetos de un paciente infectado a los no infectados. Ellos proporcionaron la base conceptual de las enfermedades infecciosas y sentaron las bases para el desarrollo de la terapia antiinfecciosa moderna, uno de los principales avances médicos del siglo XX.
Marcus M. Reidenberg, profesor de farmacología, medicina y salud pública en el Weill Cornell Medical College de Nueva York, selecciona en este nuevo Pharmacotherapy revisited los 27 artículos que jugaron un papel determinante en el desarrollo de la terapia antimicrobiana, entre los que se encuentran, tal y como se publicaron en la época, desde los primeros hallazgos de Paul Ehrlich hasta el artículo en el que Fleming describe por primera vez los efectos de la penicilina.
La terapia antiinfecciosa moderna se inició a principios del siglo XX con Ehrlich y sus compuestos para el tratamiento de la sífilis. Domagk y su grupo desarrollaron posteriormente las sulfonamidas usando sus mismos métodos sistemáticos en los años 30. Fleming, por su parte, observó las zonas de inhibición de crecimiento bacteriano alrededor del moho en una placa de cultivo y averiguó que el moho estaba segregando una sustancia antibacteriana. Pero tuvieron que pasar diez años hasta que Chain y Florey probaran esta secreción en ratones infectados.
La razón para este retraso de una década es que se sabía que los germicidas hacían más daño a los tejidos que a las bacterias y en aquel momento se asumió que la secreción del Penicillium era también un germicida. Un error que retrasó su desarrollo durante diez años y que confirma el dicho de que es mejor no saber nada que saber algo erróneo.
Reidenberg propone con esta exhaustiva selección un viaje a los orígenes y al desarrollo de la terapia antiinfecciosa moderna.
